Once your account has been created, select Book by Service: Free OT Phone Consultation.
Select any date and time. Please note that this is not a fixed appointment. This slot is for our occupational therapist to discuss your needs. You may be contacted earlier or later by e-mail if :
A) We have all the necessary information and you have been put on the waiting list or for a service.
B) We require additional information or documentation.

Once the process has been completed, you will be placed on the waiting list and contacted as soon as availability becomes available.
